This page will guide you on how to collect and submit physical samples for diagnosis, based on your particular situation. Please note some exceptions:
- For samples originating anywhere other than North Carolina, please see these guidelines for out-of-state samples.
- To submit digital, image-only samples, please login to the database, fill out the form, and attach the images to be uploaded. If your contact information is not already in our address book, you'll need to create an account there first.
